Crisp County Family Connection and community partners are conducting a community assessment to help determine the mental health and substance abuse resources, programs and activities that are needed in Cordele, Crisp County and outlying communities. A group of community leaders have worked together to develop the survey. The survey is being emailed to local citizens by partners such as the school system, local government, the faith community, and nonprofit organizations.
The survey can be accessed at https://bit.ly/CrispCounty. The survey can be completed on a smart phone, device such as an IPad, or a computer. Local organizations or churches who want paper copies of the survey can contact Sherry Evans, Family Connection Executive Director, at sevans@crispschools.org to get copies. The survey can also be accessed through scanning a QR code with a smart phone:
